After receiving feedback about our advocacy referral process, we are delighted to launch our new-look online referral forms. We now have separate forms on our website for the following services:
- Care Act Advocacy
- Community Advocacy
- Health Complaints Advocacy
- Independent Mental Capacity Advocacy (IMCA)
- Independent Mental Health Advocacy (IMHA)
This is to help make it easier to know which service to refer to when working with someone who needs to be heard. We have also updated the main Referrals page, including links to each form as well as referral forms for our Bradford and Craven Autism AIM, Leeds Autism AIM and Leep1 projects.
How to refer for advocacy
If you want to make a referral to any of our services, please visit our Referrals page. Then, you will see a description of what each service does. Click on the button for the service you need and fill in the form. After that, we will be in touch within two working days.
