Feedback on our services
We have a new feedback form covering all our services – advocacy, Asking You and Autism AIM. The form is in Easy Read and can be used for the service you accessed most recently.
There is a drop-down menu, asking which service you accessed. Next, there is a question asking to give our service a rating out of 10, with 10 being the highest. Then, there are questions asking what you liked and did not like about it.
Any feedback given in the survey will help us to improve what we do for people in Leeds and beyond. To complete the form, please click on the button below.
